OSHA Form 300 Requirement

OSHA Form 300 is a form that employers who are subject to OSHA’s recordkeeping rule must use to record information pertaining to specific, work-related injuries or illnesses. On this form, employers must record when and where the injury or illness occurred, the nature of the case, the name and job title of the employee injured or made sick, and the number of days away from work or on restricted or light duty, if any.

Who Must Complete OSHA Form 300?

To be subject to the OSHA Form 300 recording requirements, employers must meet both of the following two requirements:

  • The employer must be in an industry that is not partially exempted from the recordkeeping requirements; and
  • The employer must have more than 10 employees.
